Today morning, one Bangladeshi expatriate of 46 years of age visited me. It was his second visit in the last 10 days; he came to me on 21st November for the first time. At the first visit he came with the complaints of mild fever, anorexia, malaise and weakness. On examination, I didn’t find any significant signs except a raised blood pressure of 150/100 mmHg. He had normal body temperature, his throat, nose and ears were normal and he had a clear chest. I gave him some symptomatic treatment.
Today, his main complaints were severe weakness, lethargy and marked anorexia. But when he entered my chamber, it appeared to me that he was having shortness of breath (SOB). However, on questioning, he denied having any SOB at home. His only concern was he couldn’t eat and felt too much weakness. Currently, he was free from fever, cold and cough.
I checked his vitals and those were as follow: Pulse: 101 beats per min; blood pressure: 130/80 mmHg; respiratory rates: 24 breaths per min; oxygen saturation: 94% on room environment; On chest auscultation with the stethoscope: there were abnormal sounds in his right side of the chest. There are certain kinds of abnormal sound found in a diseased lung. In my patient, the sound heard was called crepitation, which is mostly found in a patient with pneumonia. And last but not the least his blood sugar was 354 mg/dl!!
His oxygen saturation was less than the normal one. He has clinical evidence of pneumonia. So, I advised for one chest X-ray. And as suspected, the features of radiological pneumonia were also present. One important feature revealed at that point. His oxygen saturation came down to 85% when he came walking to my chamber after doing the X-ray. For doing X-ray he needed to go down a floor, using the stairs. When came up, i.e. when he underwent some kind of exertion, his oxygen saturation dropped. It indicated that his lungs damage were significant.
He had all the features suggestive of COVID19. The thing remained was to do the confirmatory RT-PCR test from his nasal swab. But he couldn’t do that. Anyway, I talked with a doctor from the regional government hospital, informed all the detailed about the patient and gave treatment according to her suggestion.
I was able to achieve 10k plus steps 🚶♂️ before 10pm. Here is the screenshot.
